£25 per hour (£35 per hour bank holidays)

Self-Employed. Vehicle required (Business Vehicle Insurance), DBS on update service (or ability to pay for DBS).

Flexible, agreed directly with client, 2hrs minimum – 12hrs maximum

About the Role
We are recruiting experienced Outcomes and Intervention Practitioners to join our network of self-employed professionals. You’ll work directly with individuals in their homes and communities, delivering personalised support that builds independence, stability, and long-term progress.
This role offers genuine flexibility — you choose whether to work with adults, children and young people, or both, based on your experience and preference. Skills are transferable across both, and we’ll match you to clients whose needs align with your expertise.
You’ll be working with people who often have complex and overlapping needs, including:
- Mental health challenges
- Complex behavioural needs
- Neurodevelopmental conditions (autism, ADHD, and related diagnoses)
- Care leavers and young people at risk
- General community support needs
Many of the individuals you’ll support will have experienced trauma, instability, or repeated breakdowns in previous support arrangements. Your role is to bring consistency, structure, and meaningful engagement.
What You'll Be Doing
- Providing strength-based, trauma-informed support and building trusting relationships with the people you work with
- Delivering one-to-one support sessions, interventions, and signposting to help individuals build independence
- Implementing support plans, risk assessments, care plans, and behaviour support plans as agreed with the client and coordinating team
- Attending appointments alongside individuals, including health, housing, benefits, and specialist services
- Supporting individuals to understand tenancy documentation, manage benefits, and engage with statutory services
- Identifying knowledge gaps and contributing to support plan reviews
- Upholding high standards of safeguarding awareness for the individuals you support, their families, and the wider community
- Maintaining clear, professional records of your work
What We're Looking For
- Minimum five years’ experience in support work, intervention, or a directly related role
- Demonstrable experience working with individuals presenting complex needs (mental health, behavioural, neurodevelopmental, or youth at risk)
- Confident working independently and managing your own caseload
- Strong understanding of safeguarding, trauma-informed practice, and person-centred support
- Excellent communication skills and the ability to build trust with people who may have been let down in the past
- Reliability, professionalism, and a genuine commitment to outcomes-focused support
Requirements
- Self-employed status (sole trader or limited company)
- Full UK driving licence and access to a vehicle with business insurance
- Up-to-date mandatory training (safeguarding, manual handling, first aid, and any role-relevant training)
- Enhanced DBS certificate, enrolled on the DBS Update Service (or willing to obtain and enrol)
- Public liability and professional indemnity insurance
- Right to work in the UK
