CHIRYŌ Outcomes & Intervention Services provides children and adults with structured, community-based support built around a coordinated, multi-disciplinary model.
At the centre of every support plan is an Intervention Practitioner, who delivers consistent, day-to-day support within the home and community. This includes practical assistance, advocacy, attending appointments, implementing strategies, and providing stability.
Where additional needs are identified, we introduce the appropriate specialist. This may include mental health assessments from Community Psychiatric Nurses (CPNs), therapeutic support from counsellors, financial guidance from debt advisors, behaviour specialist, or input from speech and language specialists.
This layered approach ensures individuals receive the right level of support — from foundational intervention through to specialist expertise — all working towards clear, measurable outcomes.
